SCENARIO IN AIDS INFECTION

A man was married for 20 years and was the father of three children. Twelve years ago, his wife died in childbirth. The baby lived and is now 12 years old. All the children are healthy.
                            
The man lived alone with his children until he met a young woman seven years ago. They slept together for several months before they broke up. He has never seen her since that time and believes that she went back to the highlands.

In the last year or so, he has not been feeling well. He is a business man in Lae and able to buy good food for his family. He eats well but has been losing weight badly over the last twelve months.

A friend has suggested that he have an AIDS test but he feels that this is not necessary. He has lived with his family alone for almost seven years. What conclusions can we draw?

Did his wife die of AIDS 12 years ago? Probably not. If she had, she would most likely have infected all her children. But all are healthy including the baby born to his mother who died.

Did the second woman have the virus? Possibly. But she has gone and there is no way of checking.

Does the man have AIDS? Possibly. He may have caught the virus from the second woman.

Are there symptoms of AIDS? Yes. He is losing weight. But that may be caused by other health problems such as tuberculosis. The body becomes depleted of nutrients as the small intestine ceases to operate.

There is depletion of protein and glucose. Protein is obtained by the body through eating its own muscles. The sufferer starts to look wasted rather than skinny.

Why is weight loss a symptom of AIDS? The virus attacks the small intestine slowly stopping the absorption of nutrients, salts and water.  The small intestine is moving to destruction by about the seventh year

Of what significance is weight loss? The man met the second woman about seven years ago. If they had unprotected sex, the man would have been infected seven years ago.

Does the man have problems? He has big problems. He is moving towards final AIDS. He has not been taking ARV drugs which means that his body is dying. Without ARV, a person with HIV/AIDS may live for 10-12 years. He has three years to go to 10 years.

When should ARV drugs be taken? Drugs should have been taken by the man 6 years ago. His gut and body tissues are wasting. There is not time to waste now. It may be too late for ARV. But he has to be tested NOW if he is to be given a chance to live.
