CIRCUMCISION AND HIV TRANSMISSION

This is an issue surrounded by controversy. But there
are basic facts to circumcision. We have already covered
the existence of mucosa cells in mouth and anus. These
are to swallow or engulf germs and viruses.

These mucosa cells exist also in the vagina and inside the
foreskin of the penis. But there is a problem when the
mucosa cells swallow the HIV virus that has been trapped
by the langerhan cells.

The HIV uses the mucosa cells to gain entry to the body as
these cells are part of the lymphatic system.

The virus moves first from the genital mucosa cells into the
lymph vessels in the groin. A sign of HIV infection is swollen
lymph nodes.

The way to block entry of the HIV is to remove the foreskin
by proper medical circumcision. Not all cultures circumcise to
remove the foreskin completely. Not all circumcised men are
protected.

But the story is not complete. A number of years ago, AIDS
Holistics came under criticism for talking of circumcision to
block HIV infection.

The view was that men will think they are safe from infection
once circumcised.

Studies in sub-Sahara Africa show that properly circumcised
men have reduced chances of infection but promiscuous
circumcised men are still HIV infected.

This is because there are two doors to HIV infection. The
first door is closed by proper medical circumcision with the
circumcision wound given time to heal.

The second door will remain open through wounds on the
penis including rashes, pimples and sores, cuts and genital
herpes.

Women have mucosa cells inside the lining of the vagina.
But they can not be circumcised. Greatest accumulation
of mucosa is on the cervix.

For men/women with STI infections, greatest accumulation of
HIV is in the genitals. This is because the STIs attract a large
accumulation of floating CD4 cells which in turn attract the HIV.

The three massive accumulation points for HIV are the genitals,
anus and gut that hold the greatest accumulation of CD4 cells
that are intended to block entry of infections including HIV.
